Compliance Inspector (CI) Jessica Ibbotson conducted an investigation regarding the allegation that Child A had a busted lip and Parent A was not notified. After conducting the investigation, CI Ibbotson finds this allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
AND
5 CSR 25-500.192(5)(B) which states: "Information regarding the date and circumstance of any accident or injury shall be noted in the child's record."
On June 8, 2023, CI Ibbotson interviewed Caregiver McWilliams who stated that Child A had fallen while running outside approximately one month ago and hit her lip on the ground. She stated that she immediately checked Child A for any injuries but did not observe any. She stated that Child A continued to play. Caregiver McWilliams stated that she did not inform Parent A of the accident or complete an accident/injury report due to the lack of any observable injuries.
CI Ibbotson called Director Abdulhafiz who stated that she was unaware of Child A falling while running outside but had never observed any injuries to her lips. She stated that staff were aware to complete accident/injury reports, regardless if any marks were left or not and to notify the parent immediately but would remind staff for future reference. She stated that there were no accident/injury reports regarding Child A falling and hitting her lip.

The facility shall conduct a staff meeting to review the center's policies and all licensing rules and regulations regarding accidents and injuries. 5 CSR 25-500.192 Health Care (5) Accidents, Injuries and Emergency Medical Care. (A-C). The facility must provide the Child Care Compliance Section with a statement, signed by all staff (paid employees or unpaid volunteers), to indicate they understand and agree to follow those rules and regulations.
