On June 9, 2023, the Office of Childhood (OOC) received an allegation that on June 8, 2023, Parent A (parent of Child A) was not properly notified after Child A (8-years-old) chipped their tooth. After conducting an investigation, Compliance Inspector (CI) Ian Fyfe has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.192(5)(A), which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
AND
5 CSR 25-500.192(5)(B), which states: "Information regarding the date and circumstance of any accident or injury shall be noted in the child's record."
On June 22, 2023, Compliance Inspector (CI) Ian Fyfe conducted an unannounced inspection to the facility and interviewed staff members Latoya Brent, Kristen Brent and Marla King. Child A was present at the facility but was not able to be interviewed as they were napping. On June 8, 2023, Child A was involved in an accident that resulted in Child A chipping their tooth. No staff members that were present during the incident observed what happened. Child A did cry after the incident. Parent A was not notified. An accident report was not created for the incident. Parent A was informed of what happened when they came to pick up Child A later that day.
On June 28, 2023, CI Fyfe conducted an interview with Parent A (parent of Child A). On June 8, 2023, Child A chipped their tooth at the facility. No staff members contacted Parent A regarding the incident and Parent A never received an accident report. This incident was the only concern that Parent A had regarding the facility. Child A still attends the facility at this time.
