Conclusion Summary
Compliance Inspector (CI) JD Greer conducted an unannounced investigation of the allegation that Child A (2 years old) was standing alone outside of the facility's back door crying. The allegation is substantiated based on the following information: 5 CSR 25-300.040 (1) "Supervision and basic care shall be provided at all license-exempt facilities." On August 16, 2023, Compliance Inspector (CI) JD Greer interviewed Assistant Director Stacy Clevenger, who stated on August 15, 2023, at approximately 8:45 a.m. she was speaking with Director Wendi Fleming in the office. Parent G said they saw a 2-year-old child standing alone outside the facility's door and the child was crying. Parent G brought the child back into the building to their classroom. After conducting an investigation, it was discovered the child had slipped out the door as other parents were dropping children off in the morning. The child was outside of the facility from approximately 8:40 a.m. to 8:45 a.m. On August 16, 2023, CI Greer interviewed Caregiver Janice Kuskey, who stated she was in Child A's classroom on the morning of August 15, 2023. At approximately 8:40 a.m., she informed Caregiver Smith-Williams she was taking a group of six boys to the restroom leaving Caregiver Smith-Williams with 8 children. As she was returning to the classroom with the six boys, she saw Child A being walked into the building by Parent G. She remembers seeing Child A in the classroom before lining the children up at the door to take them to the restroom. She does not have any supervision concerns with Caregiver Smith-Williams. She is not aware of a child getting out of the building before. On August 16, 2023, CI Greer reviewed an incident report for Child A from August 15, 2023. The incident occurred at approximately 9:45 a.m. Wendi Fleming and Stacy Clevenger were in charge of the facility at the time. Caregivers Janice Kuskey and Marlisa Smith-Williams were responsible for Child A. Parent G made Wendi and I (Clevenger) aware of Child A standing alone outside the south doors. Upon questioning teachers about the incident, we were informed Child A must have followed another parent outside while Ms. Janice was in the restroom with other students. Ms. Marlisa was in the classroom talking with other parents.
