Conclusion Summary
On August 24, 2023 the Office of Childhood received a report alleging the facility has mold issues. It is in and around the vents throughout the building. The facility supposedly had a person come clean them last Friday (August 18, 2023); however this did not resolve the issue. After conducting an investigation, Compliance Inspector (CI) Caylin Mertz has found this allegation to be substantiated based on the following evidence and rule violations: 5 CSR 25-500.082(2)(A)9 which states "The facility shall be clean at all times and free of dirt, insects, spiders, rodents or other pests." AND 5 CSR 25-500.082(1)(A) which states "The premises shall be safe and suitable for the care of children." On August 24, 2023 CI Mertz conducted an unannounced inspection at the facility. CI Mertz observed each of the floor registers and adjoining duct work. A white porous, mold like substance was observed in the ductwork and at least one adjoining floor register which children were observed crawling on. Director Cassie Shaddox, Caregiver Lindsey Stiner, and Head Start Supervisor Tawnya Pace were interviewed. Caregiver Lindsey indicated there is mold in several of the ducts. Director Cassie and Supervisor Tawnya indicated the facility is in need of sump pump which is causing water to sit in the ducts and results in mold growth. At least some of the ducts were cleaned previously but the facility will be closing at the close of business on August 24, 2023 until the ducts can be inspected and cleaned sufficiently. On September 8, 2023 CI Mertz observed a sanitation inspection dated September 6, 2023.The inspection indicates the facility fails to conform with sanitation requirements of the Missouri Department of Health and Senior Services. The noted deficiencies include dirt and debris build up inside the HVAC ductwork that is visible when you take off the floor registers as well as potential evidence of mold observed inside the HVAC ductwork that is visible when you take off the floor registers. There was also some growth observed to the underside of one of the floor registers.
