Conclusion Summary
On September 6, 2023 the Office of Childhood received a report alleging "On June 8, 2023, Child A was injured when a freestanding shelf fell and hit his face." After conducting an investigation, Compliance Inspector (CI) Caylin Mertz has found this allegation to be substantiated based on the following evidence rule violations: 5 CSR 25-500.082(1)(A) which states "The premises shall be safe and suitable for the care of children." On September 13, 2023 CI Caylin Mertz conducted an unannounced inspection. A walkthrough of the facility was conducted. Director Heather Conley indicated Child A did pull a shelf over on himself. An incident report was completed by Caregivers Regan Burns and Savannah Tracy. They no longer work at the facility. CI Mertz observed an incident report stating "Child was either climbing or pulling on shelf and it fell as bookshelf was falling it grazed his face on the way down." The report is signed by Parent A and dated June 8, 2023. CI Mertz observed the shelf that Child A pulled over on himself. It was observed unmounted to the wall at the time of inspection. The wooden shelf measures approximately two feet tall and three feet wide. The shelf was pushed back against a room divider.
