Compliance Inspector (CI) Jessica Ibbotson conducted an investigation regarding the allegation that Child A and Child B had been coming home with unexplained bite marks on multiple occasions without receiving accident/injury reports or staff being aware of the biting incidents. After conducting the investigation, CI Ibbotson finds this allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
AND
5 CSR 25-500.192(5)(B) which states: "Information regarding the date and circumstance of any accident or injury shall be noted in the child's record."
On September 18, 2023, CI Ibbotson interviewed Provider Schroeder, Caregiver Flores and Caregiver Collins. All staff stated that they did not complete accident/injury reports for any of the children. Staff also stated that they did not inform parents immediately after an accident and/or injury, but would notify at pickup, regardless of what time the accident occurred. Provider Schroeder and Caregiver Collins stated that parents were never notified immediately unless the injury was more severe and needed medical attention. All staff stated that Child A and Child B were bit often by other children but parents were not notified until pickup. Provider Schroeder stated that she was aware of the biting incidents regarding Child A and Child B and never reported to Parent A or Parent B that she was not. She stated that she provided an informational sheet to the parents of the children who were biting and communicated with the children about having a "nice mouth" and reminding them of no biting.

The facility shall notify all staff members of the violations which were substantiated, and specify the actions to be taken by all staff in order to comply with all violations cited. A copy of the memo, letter, or staff meeting agenda shall be submitted to the Office of Childhood.
