Conclusion Summary
Compliance Inspector (CI) Heather Radney conducted an investigation of the allegation that Child A came home with scratches going all directions on his face two to three inches long. No one notified parent. When parent asked what happened, she was first told they didn't know and the scratches could've come from a fork. Later, the parent was told that Child A got into a fight with another child. The allegation is substantiated based on the following information: 5 CSR 25-500.192 (5) (A) which states "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions. On September 28, 2023, CI Radney interviewed LLC Member Leah Martin and Director Erica Henderson. They reported that Caregiver Margaret Riley did not notify Parent A immediately of scratches Child A received to his face when his own fork scratched him. On September 28, 2023, CI Radney attempted to reach Margaret Riley by phone. A voicemail was left but no return call was received. CI Radney attempted to reach Parent A by phone. A voicemail was left but no return call was received. On September 28, 2023, CI Radney received and reviewed a statement written by Margaret Riley from Little Learners Christian Academy II which documented the injury and was located in Child A's file. The report read "On Tuesday August 29th we were eating lunch when Child A turned around looking outside to see the school bus. I asked him to please turn around in doing so he scratched himself with his fork leaving a lark mark. Later that day he was pulling another child's hair while I was changing diapers and the child scratched the other side of his face. It was very busy that day and I forgot to document what happened. I was off the next day so I didn't have a chance to speak with the mother, but on Thursday August 31st I spoke with the mom and told her what happened..." Caregiver Margaret Riley did not immediately contact Parent A when Child A scratched his face with a fork nor when another child scratched his face on August 29, 2023.
