On October 26, 2023, the Office of Childhood (OOC) received an allegation that: Injury to child on 10/13/23 was not reported immediately to parent. After conducting an investigation, Compliance Inspector (CI) Michelle Honea has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.192(5)(A)., which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On November 1, 2023, CI Honea conducted a phone interview with Parent A. The facility called her husband around 1:00pm to discuss the incident. The facility called again around 2:00pm. Child A's grandma picked up Child A that day and said the incident report shows time of incident at 11:35am. She was told that Child A had cried and was shaking when the incident happened. Director Christy Weisel said the incident actually happened around 12:00-12:30pm when the caregivers were putting the cots out. She questioned the director as to why it took so long to receive a call but was never given a direct answer. Normal naptime is around 12:00pm. She was told the cot fell onto Child A's hand and only injured Child A's pinky finger. She knows accidents happen but expect to be told immediately. Child A's only had a jammed finger with bruising which was good. She is still taking Child A to the facility.
On November 2, 2023, CI Honea conducted an unannounced inspection to the facility and interviewed program manager Angel Morrow and caregiver Jessica Owens. All caregivers stated that Jen Rosas was working on the other side of the classroom putting out cots that were leaning on the shelves. No one actually saw what happened, but it appeared the Child A grabbed the cots and the cot fell smashing Child A's finger. All caregivers stated that the finger was looked at and Jen Rosas asked for an ice pack. All caregivers stated that no one said anything to the office and there was no incident report completed. No one told parents what happened to Child A's finger at the time of incident. All caregivers stated that parents can be notified through Procare and if that is not done, staff are to notify the office, and the office will notify parents.
On November 2, 2023, CI Honea observed the classroom where the 3-year-olds were asleep on cots and the 5-year-olds were working quietly on computers. CI Honea reviewed Child A's enrollment form, caregiver Jen Rosas's contact information, and the incident report that shows the time of incident was 11:35am and Parent A was notified by phone at 1:05pm.
