Compliance Inspector (CI) Jessica Graham investigated the allegation that Child A received a bloody nose around October 13-17 and Parent A was never notified of the incident by the facility. This allegation is substantiated based on the following information.
5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On November 15, 2023, director Jill Buzzell stated that the incident with Child A (4-years-old) occurred on October 16, 2023. It was not a major nose bleed and Child A did not have any marks. Parent A was told about the incident at pick up. Since it didn't leave a mark and there was only a little blood she didn't think anything of not writing up a report.
On November 15, 2023, caregiver Jordan Harris stated that one of the kids threw a hula hoop and it hit Child A on the nose. Child A's nose started bleeding and Child A was crying. Both caregivers should be writing a report when a child gets hurt. She spoke to Parent A at pick up time and told Parent A that Child A got hit in the nose. She told Parent A that Child A's nose had a nose bleed from the incident. Parent A never asked for an incident report.
On November 15, 2023, CI Graham observed an incident report for Child A, dated October 23, 2023; however the report noted it was regarding an incident that occurred on October 16, 2023, at 11:00am. The report notes: "A friend and [Child A] were playing a good guy, bad guy game on the playground with hula hoops and the friend hit [Child A]'s nose with the hula hoop, causing it to bleed." The report notes that Child A's nose was pinched shut and she declined ice. The Notification to Parent section was blank. The report's staff signature is dated October 23, 2023.
On November 27, 2023, Parent A (parent of Child A) stated that Child A told her that she was hit with a hula hoop at school. After Child A told her about the incident, she asked Jordan about it. Jordan told her that it did happen and that she took her to Jill because Child A's nose was bleeding. She asked Jill about the incident and Jill told her that she didn't mention it because it was not a big deal.
