Compliance Inspector Jessica Graham investigated the allegation that Parent A went to the facility on January 8, 2024, around 3:30pm to pick up Child A (9-weeks-old) and she found him asleep on his side with a bib tied around his neck. There was also a loose swaddle and burp rag in the crib near the baby's face. This allegation is substantiated based on the following rule violation.
5 CSR 25-500.092(1)(B)2.G. which states: "Cribs, portable cribs, and playpens shall be free of soft objects, loose bedding, or any object that can increase the risk of entrapment, suffocation, or strangulation. Examples include bumper pads, pillows, quilts, comforters, sleep positioning devices, sheepskins, blankets, flat sheets, cloth diapers, bibs, pillow-like toys, wedges, infant positioners, special mattresses, special sleep surfaces, and other similar items."
On January 10, 2024, caregivers Margaret Thatch, Pam Murphy and Deshiya Shannon all agreed that Child A, age 2-months-old, was laying in the pack n play with a bib on while sleeping. Deshiya laid him down forgetting to take off the bib. She placed him on his back in the pack n play and he was in a sleep sack that was provided by Parent A. They all stated that nothing was covering Child A's face while he was laying down.
Parent A said that Child A was swaddled in the pack n play, laying on his side, with a bib covering his face when she arrived to pick him up.
