Compliance Inspector (CI) JD Greer conducted an unannounced investigation of the allegation that on January 26, 2024, Child A (20 months old) was bitten on the face. Parent A (Parent of Child A) was not notified immediately after the bite happened. The allegation is substantiated based on the following information:
5 CSR 25-500.192(5)(A) which states, "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On January 31, 2024, CI Greer interviewed Parent A, who stated on January 26, 2024, Child A was bitten on the face. She was not notified of the incident until pickup. She did not receive a call from the facility.
On January 31, 2024, CI Greer interviewed Director Kim Sunjaro, who stated on January 26, 2024, Child A was bitten by another child at the facility. Parent A was not notified of the bite. Parent A was not called.
On January 31, 2024, CI Greer interviewed Caregiver Sophia Clapp, who stated she was in Child A's classroom on January 26, 2024. She saw the bite occur and immediately notified the office. That evening at pick-up, Parent A said the office did not notify her of the bite. The bite occurred between 2:00 p.m. and 3:00 p.m. Parent A picked up at approximately 4:30 p.m.
On January 31, 2024, CI Greer observed an incident report for Child A dated January 26, 2024 and showed the time of the incident as 2:56 p.m.
