5 CSR 25-300.050 (11) In case of an accident or injury to a child, the provider shall notify a parent immediately. If a child requires emergency medical care, a parent's prior written instructions shall be followed. A form shall be completed indicating the circumstances and the date and time of the injury. The form shall be signed by the caregiver and his/her supervisor. A copy of the form shall be given to the parent the day of the accident or injury and necessary explanations shall be given. The form shall be filed in the child's record.
Conclusion Summary
Compliance Inspector (CI) Heather Radney conducted an investigation of the allegations that Child A (14 months old) came home with a bite mark on his chin January 26, 2024. Parents of Child A texted the directors that evening with pictures asking for an incident report. As of today February 2, 2024 the parents have received no communication in regard to this incident. The allegations are unsubstantiated based on the following information: 5 CSR 25-300.050 (11) - In case of an accident or injury to a child, the provider shall notify a parent immediately. The form shall be filed in the child's record. On February 8, 2024, CI Radney conducted an unannounced inspection of the facility. CI Radney observed the caregivers to be providing basic care and supervision to children. CI Radney did not observe any children biting and did not observe any children injured. On February 8, 2024, CI Radney conducted interviews with Caprice Law, Destiny Peterson, Donna Campbell, Keyshawn Frink, Liz Almanzar, Monica Bruton and Shantella Bean. Keyshawn Frink reported she was not there when Child A was bitten on the chin but she knew it happened. Shantella Bean reported Child A was bit by Child B because he had a toy she wanted. Child B bit him on the chin. She was in the room when the incident happened. She did not fill out an incident report, the other teacher did. She did not notify Parent A. On March 12, 2024 and March 21, 2024, CI Radney attempted to reach Parent A. A voicemail was left but no return call was received. On March 22, 2024, CI Radney attempted to contact Dashayla Bean. A voicemail was left but no return call was received. On April 1, 2024, CI Radney attempted to contact Leila Leville. There was no answer and no voicemail option.
