On April 9, 2024, the Office of Childhood (OOC) received an allegation that Parent A's (parent of Child A) daughter was picked up from school with a bump on her head and the teacher said it happened around 12:00pm. The child was not picked up until 5:00pm and Parent AA was notified at that time. After conducting an investigation, Compliance Inspector (CI) Jade Stewart-Martin have found this allegation to be substantiated based on the following evidence:
5 CSR 25-300.050 (11)., which states: "In case of an accident or injury to a child, the provider shall notify a parent immediately. If a child requires emergency medical care, a parent's prior written instructions shall be followed. A form shall be completed indicating the circumstances and the date and time of the injury. The form shall be signed by the caregiver and his/her supervisor. A copy of the form shall be given to the parent the day of the accident or injury and necessary explanations shall be given. The form shall be filed in the child's record."
On April 22, 2024, CI Stewart-Martin conducted an unannounced inspection and conducted interviews with director Tiffani Martin and teachers: Kendall Miller and Kyrah Martin. Child A (age 20-months-old) fell off the toilet while potty training and bumped her head. Kyrah Martin stated that Child A had a knot forming after Child A fell off the toilet. Kyrah Martin attended to Child A and got ice for her head and monitored her for the rest of the day. There was an incident report filled out and the director was notified. Tiffani Martin did not notify the parents immediately after the incident because Child A was acting fine, and there were no obvious marks. Kendall Miller was the teacher who notified Parent AA (parent of Child A) of the incident at pick up around 5:00 P.M. Parent AA did not receive the incident report because Ms. Miller could not find it.
On April 22, 2024, CI Stewart-Martin observed Child A's enrollment form and the incident report filled out on April 5, 2024, by Kyrah Martin regarding Child A. The incident report documented that Child A hopped down off the potty and fell and hit her forehead and had a knot and ice was placed on her forehead for a few minutes. The injury occurred at 11:45 am but did not state when or how the parents were notified and was not signed by Parent A or Parent AA.
On April 26, 2024, CI Stewart-Martin conducted a phone interview with Parent A. Child A had a head injury that occurred on April 5, 2024, at approximately 12:00 P.M. Parent A was not notified of the incident. When Parent AA picked up Child A at approximately 5:00 P.M., staff told him that Child A had incurred an injury, but no details were given. Parent A had to call the center and to get details about the incident. At this time the director said that Child A had fallen off a toilet while the staff was potty training Child A. Parent A took Child A to the hospital, where the findings were just a contusion, but Child A did have a large bruise from the injury. Parent A did not receive the incident report or sign an incident report.
