On April 9, 2024, the Office of Childhood (OOC) received an allegation that on April 8, 2024, Child A (2-years-old) had an altercation with another child(ren) and came home with bruises and scratches all over his face, hands, and under his chin. The daycare did not file an incident report and the staff members did not appear to know about the altercation. After conducting an investigation, Compliance Inspector (CI) Ian Fyfe has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.192(5)(A), which states that: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
AND
5 CSR 25-500.192(5)(B), which states that: "Information regarding the date and circumstance of any accident or injury shall be noted in the child's record."
On April 23, 2024, Compliance Inspector (CI) Ian Fyfe conducted an unannounced inspection to Strawberry International Schools of St. John and interviewed Director Shawiesha White and 2-year-old Caregiver Jodi Enloe. Ms. White was familiar with the reported concerns, but Ms. Enloe was not. On April 8, 2024, Child A was involved in an incident in the 2-year-old classroom in which another child scratched him on his face. Staff did not notify Parent A of this injury to Child A. Ms. White stated that she did not see any injuries on Child A prior to him leaving for the day but did see scratches on his face the following day when he returned to the facility and Parent A had contacted Ms. White once she returned home on April 08, 2024 and asked how Child A got his scratches on his face. An accident report for this incident was not present in Child A's facility record. Parent A (parent of Child A) was not notified of the incident after it had occurred. Child A is still attending the facility at the time of this conclusion.
On April 24, 2024, and on April 30, 2024, CI Fyfe attempted to interview Parent A (parent of Child A) via phone call but was unsuccessful in the attempts.
