On April 29, 2024, the Office of Childhood (OOC) received a report alleging that one of the ceiling tiles was really nasty again. They probably have a leak or something they should fix but they say they have other important things to work on. After conducting the investigation, Compliance Inspector (CI) Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.082(2)(A)6. Which states: "Walls, ceilings, and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces. ."
On May 2, 2024, CI Eddins interviewed Caregiver Brenda Davis, Caregiver Fanta Koumare, Director Jenifer Reiter, Caregiver Samantha Riker, and Caregiver Samantha Shoemaker. All staff members provided consistent information. All staff members have observed small water spots from a leak in the facility's roof on one or two ceiling tiles in Infant Room B. Jenifer Reiter stated that she has also observed a leak which was located in the utility closet near the front office. The leaks have been occurring since the beginning of April 2024 due to the heavy rain.
On May 2, 2024, Compliance Inspector (CI) Cole Eddins conducted a walkthrough of the facility and observed a utility closet that had one ceiling tile with two water stains located on the left center of the tile. CI Eddins did not observe any signs of mold or dripping water. CI Eddins also observed two ceiling tiles with water stains in Room B. Both water stains on the ceiling tiles in Room B were three to six inches in diameter with no signs of mold or dripping water.
On July 1, 2024, CI Eddins observed the Bureau of Environmental Health Services Sanitation Inspection Report dated May 16, 2024. The report showed the following deficiency noted: "Ceiling tiles in Staff Office and in Infant Room B have water damage."
