On April 29, 2024, Office of Childhood (OOC) received a report alleging that the carpet square always had pieces of dirt, leaves, etc. It was maybe vacuumed once a week. On April 22, 2024, reporter picked up Child A (10 months old), who is now crawling, and she was almost black on the front of her outfit. Reporter is not sure if the floor ever gets mopped. The exersaucers have dirt/food caked on them as if they are never cleaned. On April 24, 2024, Child A was on the ground, trying to pick up a Cheerio that she could choke on. The bottle/food/sink area is filthy and looks like it is never cleaned. There is a thin blanket that lays on top of the counter and it is visibly gross. After conducting the investigation, Compliance Inspector (CI) Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.082(2)(A)9. which states: "The facility shall be clean at all times and free of dirt, insects, spiders, rodents or other pests."
and
5 CSR 25-500.082(2)(B)2.A.(II) which states: "A crawl area protected by stable dividers shall be provided. The area shall have a resilient and washable floor covering which shall be mopped and sanitized daily. Washable area rugs that are laundered daily may be used.
and
5 CSR 25-500.082(2)(B)2.B.(VI) which states: "Floors shall be tile, linoleum or wood and shall be mopped and sanitized daily. Washable area rugs that are laundered daily may be used. Centers initially licensed after the effective date of these rules or facilities adding new infant/toddler space shall not use carpet;"
On May 1, 2024, CI Cole Eddins conducted an unannounced inspection and interviewed Assistant Director Shanay Smith who stated that Caregiver Jewel Hart and Caregiver Markenya Dehart did not do a great job at cleaning their classroom. She has observed a dirty sink area where the infants' bottles and cups were washed. She observed a dirty sink, dirty toys, a food-covered Exersaucer, unswept tiled floors, and a dirty rug in Child A's (11-month-old) classroom. Jewel Hart and Markenya Dehart do not wash blankets often and did not clean bottles as well as they should. They left the sink area cluttered and unclean for long periods. Jewel Hart and Markenya Dehart only vacuumed the rug about once a week when they should have vacuumed the square rug daily.
On May 1, 2024, CI Eddins conducted a walkthrough of the facility and observed Child A's infant room to have a cluttered bottle-washing sink area. CI Eddins observed Child A's classroom tiled floor to have some leaves and food on the ground. CI Eddins observed a dirty exersaucer that was taken apart and had food and dirt caked on the top rim of the tray. The rug appeared to be dirty and not vacuumed properly.
On May 1, 2024, CI Eddins interviewed Parent A over email, and she did not provide any additional information that was not already listed in the report.
5 CSR 25-500.202(2)(B) which states: "The written diet plan for each infant/toddler shall be followed. The parent(s) shall update the plan according to the individual needs of the child until they grant permission in writing for the child to eat table foods."
On May 1, 2024, CI Eddins interviewed Assistant Director Shanay Smith who stated that Child A's feeding plan did not include an update for caregivers to provide Child A with infant food or table food. Child A only had his formula recorded under "type of food" on his infant and toddler feeding and care plan located in Child A's file.
On May 1, 2024, CI Eddins observed Child A's infant and toddler feeding and care plan sheet which did not include what types of infant or table foods Child A could have.
