Conclusion Summary
On May 10, 2024 at approximately 8:03 AM, Child A ( age 4-years-old), was left outside in the outdoor play space for approximately 10 minutes before returning inside independently via the exit door in Classroom E. 5 CSR 25-500.182(1)(A)1. which states: "Caregivers shall not leave any child without competent supervision." AND 5 CSR 25-500.182(1)(A)3., which states: "Caregivers shall provide frequent, direct contact so children are not left unobserved on the premises." On May 14, 2024, caregiver Kylie Fudge, Megan Zieres and director Brittanie Sebastian said that Child A, age 4-years-old, was left unattended outside on the playground after Kelsey Kehr, caregiver, had brought her class inside. This happened in the morning after outdoor play. Kylie helped Kelsey bring the children inside the building and neither caregiver did a head count to make sure all the children were present. Child A walked into the building herself from the playground and no one was aware she was missing until she came into the classroom. Kylie and Megan both stated that Kelsey did not ever do head counts for her class. All caregivers were aware that Child A was a wanderer and did not like to line up. Child A said that she was outside by herself one time. She was in the caterpillar and heard her teachers say to line up but she didn't want to go inside. She went inside by herself. On May 28, 2024, Parent A said he was made aware of the incident with Child A and understands that things can happen. He had no concerns with the facility. CI Graham called Kelsey Kehr, caregiver, but no contact was made. While at the facility on May 14, 2024, for the investigation, Kylie Fudge informed CI Graham of an incident that occurred that morning with Child B, age 4-years-old. Kelsey had taken her class outside and Child B was left in the classroom asleep on the carpet. Kylie found him in Room E on the carpet sleeping when she went in the room to get her water. Kelsey had Child B on her attendance sheet but stated that she was not even aware he was there that day. Child B was alone in the room for approximately 30 minutes before Kylie found him. CI Graham attempted to contact Parent B but was unsuccessful.
