On May 14, 2024, the Office of Childhood (OOC) received a report alleging that on May 9, 2024, Child A (3 years old) received a bruise and knot on his forehead while at daycare. When Child A's mom picked up Child A, she asked the director what happened, and director didn't know but gave the mom five possibilities of what could have happened. No one notified the parents when the injury occurred. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.192(5)(A) which states, "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On May 17, 2024, CI Eddins attempted to interview Parent A. Parent A did not respond. CI Eddins was unable to leave a message due to the voicemail being full.
On May 17, 2024, CI Eddins conducted an unannounced inspection and interviewed Director Julita Harris, Caregiver Kirsten Neessen, and Caregiver Makenzie Centeno. Kirsten stated that she heard that Child A fell and hit his head. Julita and Makenzie provided consistent information. They stated that on May 9, 2024, Child A was leaning over a sit-and-spin, and he toppled over it and hit his head on the floor. The accident caused a bruise on the middle of his forehead. Parent A was not notified of the accident.
On May 17, 2024, CI Eddins observed Child A's accident report, which was dated May 9, 2024. The report shows the time the incident happened was at 11:30 a.m. The report stated that Child A was leaning over a sit and spin. He toppled over it and bumped his head on the floor. A cold cloth was applied to Child A. CI Eddins did not observe a parent's signature on the report. Under notice to parent, it says Julita notified by phone at 6:35.
On April 6, 2024, CI Eddins attempted to interview Parent A. Parent A did not respond. CI Eddins was unable to leave a message due to the voicemail being full.
