Conclusion Summary
On May 28, 2024 the Office of Childhood (OOC) received a report alleging "Within the last several months, there have been three times the Director/Owner has stated they would not provide meals, with only one or two business day notices." After conducting an investigation, Compliance Inspector (CI) Caylin Mertz has found this allegation to be substantiated based on the following evidence and rule violation: 5 CSR 25-500.202(1)(A) which states "The provider shall supply and serve nourishing food according to the Meal and Snack Food Chart provided in this rule." On June 4, 2024 CI Mertz conducted an unannounced inspection at the facility. A walkthrough was conducted. CI Mertz interviewed Manager Valerie Piggie, Director Sharon Hansen, Caregiver Yughonda Dotson, Caregiver Nicole Fleischmann and Caregiver Wanda Johnson. Each stated there was a time at the end of May when parents were asked to provide meals for one week. Director Sharon explained their cook came in on May 28, 2024 and went for break at 10:00 a.m. and did not return. She asked parents to bring breakfast and lunch for the remaining days of that week; Wednesday, Thursday and Friday. The facility provided milk and the afternoon snack. All other caregivers interviewed provided corroborating statements regarding meals. On July 11, 2024 CI Mertz left a voicemail for Parent A, Parent B and Parent C. Parent D was interviewed. She indicated they were asked by the facility to provide meals for a week in May. The facility did not tell her what types of foods to pack.
