On May 29, 2024, the Office of Childhood (OOC) received a report alleging that Child A's (4 years old) medication form was not filled out by daycare. Compliance Inspector (CI) Heather Radney conducted an investigation and found the allegation to be substantiated based on the following information:
5 CSR 25-500.192(3)(I) - The date and time(s) of administration, the name of the individual giving the medication and the quantity of any medication given shall be recorded promptly after administration. This information shall be filed in the child's record after the medication is no longer necessary.
On June 5, 2024, CI Radney received and reviewed the Medication Authorization form for Child A. The date medication is to be given is listed as May 17, 2024-June 14, 2024. The first time the administration is listed is May 23, 2024 by Samantha Determan.
On June 5, 2024, CI Radney received and reviewed Child A's attendance record. Child A attended May 21-24, 2024 and May 28-29, 2024.
On June 5, 2024, CI Radney conducted interviews with Alisha Vallance, Lea Wilcox and Samantha Deter. On July 9, 2024, CI Radney conducted an interview with Melanie Munder. Samantha reported she administered Child A's medication. The first two days she didn't know there was a medication form that needed to be completed. She did use Brightwheel to message Parent A that the medication had been given. Once she learned there was a form to complete each time she gave the medication, she began documenting on the form. Melanie reported that Samantha should have completed the medication form for Child A. She told Samantha about the form and where it was located. She doesn't know why Samantha didn't fill it out.
On July 8, 2024, CI Radney attempted to reach Parent A. A voicemail was left but no return call was received.
On July 9, 2024, CI Radney conducted an interview with Parent B. She provided no information relevant to the finding. On July 9, 2024, CI Radney attempted to reach Parent C by phone. A voicemail was left but no return call was received.
