On June 7, 2024, the Office of Childhood (OOC) received a report alleging that the reporter's child fell off of something and hit the left side of her head next to her temple. Reporter wasn't notified until pick up that her child hurt her head. On the accident form, all it stated was they provided TLC. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.192(5)(A) which stated: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On June 14, 2024, CI Eddins interviewed Parent A. Parent A stated that she was not notified about Child A hitting her head until the end of the day on May 28, 2024. She observed an accident report which stated that TLC was provided to Child A after the accident occurred.
On June 14, 2024, CI Eddins conducted an unannounced inspection and interviewed Caregiver Jasmine Rooney, Youth Development Director Synthia Roth, and Director Codi Green. Jasmine Rooney inferred that Child A might have hit her head while playing in the orange tunnel outside on the playground. She observed Child A crying and holding her forehead while crawling out of the orange tunnel. She wasn't sure if Parent A was notified right away. Synthia Roth and Codi Green stated that Parent A was not notified about Child A's accident right away. Codi Green stated that Parent A was not notified about Child A's forehead injury right away because she did not think the injury was a major injury. Parent A was notified of Child A's injury at the end of the day on May 28, 2024, when Parent A observed the accident report.
On June 14, 2024, CI) Eddins conducted a walkthrough of the facility and observed Child A's accident report on file and there was nothing written under the notice to parent section.
