Conclusion Summary
On June 24, 2024, The Office of Childhood (OOC) received a report alleging that if a child's parent works at the facility and the child becomes sick or injured during the parent's work shift the parent is not informed until the end of their work shift. On June 21, 2024, at pick up time Parent A (Parent of Child A) observed that Child A (2 years old) had a bite mark on his right wrist. Parent A was not informed during the day of the bite mark on his right wrist. Compliance Inspector (CI) JD Greer conducted an unannounced investigation and found the allegation to be substantiated based on the following information: 5 CSR 25-500.192(5)(A) which states, "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions." On June 26, 2024, CI JD Greer interviewed Caregiver Shelby Bailey, Parent B (Parent of Child B), and Parent A (Parent of Child A). Caregiver Shelby stated on June 21, 2024, Child A was bitten by another child. Parent B, her co-teacher, filled out the incident report. The caregivers do not notify the parents when an injury occurs. Director Kim said to just let her know when a child is injured, and she will tell the parents. Parent B, who is also a caregiver, stated on June 21, 2024, Child A was bitten in the classroom by another child. Parent B filled out the incident report immediately. Her understanding was that if there is no blood you don't have to notify the parent immediately. The incident occurred at approximately 10:50 a.m. She gave the report to Director Kim at approximately 12:00 p.m. She does not believe Parent A was notified of the bite. She notified Parent A of the bite at pick up. She can't remember exactly but she thinks pickup was between 3:00 p.m. and 5:00 p.m. Parent A, who is also a caregiver, stated on June 21, 2024, Child A was bitten, and she was not notified until the end of the day. On June 26, 2024, CI Greer reviewed an Incident Report for Child A, and observed the date and time of the incident was June 21, 2024, at 10:50 a.m. Child A is listed as the child. Parent B is listed as the staff member responsible for child at time of incident. Caregiver Shelby is listed as the other staff member who observed the incident. Description of the incident states that Child A and a friend were sitting outside when the friend bit Child A. Child A was bitten on the right forearm. Child A was brought inside, arm cleaned and given a cold towel. Child A was provided TLC as well. Notice to parent is blank and has no time or staff member listed. The report is signed by Parent A, Parent B, and Director Kim.
