On June 21, 2024, the Office of Childhood (OOC) received a report alleging that Child A (17 months old) and Child B (4 years old) did not have medical examinations on file at the facility. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.122(2)(A) which states: "The provider, within thirty (30) days following the admission of each infant, toddler or preschool child, shall require a medical examination report signed by a licensed physician or registered nurse who is under the supervision of a licensed physician and completed not more than twelve (12) months prior to admission. The provider may use the department's medical assessment form, or the provider may use its own form if it contains all the information on the department's form. The Child Medical Examination Report (Infant/Toddler/Pre-School) form, revised 2021, is incorporated by reference in this rule, as published by the Missouri Department of Elementary and Secondary Education, PO Box 480, Jefferson City, MO 65102-0480 and available by the department at https://dese.mo.gov/childhood/forms. This rule does not incorporate any subsequent amendments or additions."
On June 26, 2024, CI Eddins interviewed Parent A, and she stated that Child A and Child B do not have medical examination records on file.
On June 26, 2024, CI Eddins conducted an unannounced inspection and interviewed Director Stephanie Nash. Stephanie Nash stated that Child A and Child B did not have doctor notes on file. Medical examinations for Child A and Child B were not on file.
On June 26, 2024, CI Eddins conducted a walkthrough of the facility and observed Child A and Child B's files. CI Eddins did not observe medical examination documents in their files.
