Conclusion Summary
Compliance Inspector (CI) Jessica Graham investigated the allegation that Sherryl, caregiver, was feeding Child A (age 4 -months old), 6oz bottles with baby food in the bottles every three hours without asking Parent A or consulting Parent A about it. This allegation is substantiated based on the following rule violation: 5 CSR 25-500.202(2)(B)., which states: "The written diet plan for each infant/toddler shall be followed. The parent(s) shall update the plan according to the individual needs of the child until they grant permission in writing for the child to eat table foods." On July 10, 2024, CI Graham observed documents from Child A's file. The Infant and Toddler Feeding and Care Plan stated that Child A would receive 2oz every two hours. A doctor's note dated June 25, 2024, stated "Please give Child A 1Tbsp. of cereal/4oz bottle every 3 hours. Do no increase bottle amount over 4oz or give baby food unless otherwise directed by provider." The Child Medical Examination Report for Child A did not state any medical concerns or specialized care. On July 10, 2024, Sherryl Paxton, caregiver, stated that she was giving Child A 6oz bottles with cereal because she felt that the 4oz bottles that Parent A had requested for him was not enough. Sherryl told Parent A that Child A was still hungry and she would give him 6oz until a doctor's note was provided. Sherryl also stated that she had given Child A some peaches in his bottle because he was constipated. Parent A had not instructed her to do that. Sherryl told Parent A that she could no longer care for Child A because she was not able to feed him when he was hungry. On July 15, 2024, Parent A stated that she told Sherryl to feed Child A 4oz bottles with one Tbsp. of cereal. She found out that Sherryl was giving Child A 6oz bottles with cereal and told her to stop because of doctor's orders. Parent A also found out that Child A was given pureed fruit in his bottles without her permission or knowledge.
