On August 8, 2024, the Office of Childhood (OOC) received a report alleging that the facility has a ceiling that is "brown with some black spots" and the reporter is concerned that it could be a health/safety issue. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.082(1)(A) which states: "The premises shall be safe and suitable for the care of children."
On August 14, 2024, CI Eddins conducted an unannounced inspection and interviewed Executive Assistant Cynthia Graham, Caregiver Tanielle Ford, and Caregiver Genneta Williams. All staff stated they had observed some water spots on ceiling tiles. After observing the photos that were taken by CI Eddins, Cynthia Graham stated that the black spots could be black mold, but she was not too sure.
On August 14, 2024, CI Eddins conducted a walkthrough of the facility and observed the downstairs Elephant classroom with two ceiling tiles with water spots and black spots. CI Eddins observed water spots on ceiling tiles in another downstairs classroom and an upstairs preschool classroom.

The facility shall notify all caregivers (paid employees and paid or unpaid volunteers) of the violations which were substantiated and specify the actions to be taken by all caregivers, in order to comply with all violations cited. A copy of the memo, letter, or meeting agenda shall be submitted to the Office of Childhood.
