On September 11, 2024, the Office of Childhood (OOC) received a complaint alleging on September 4, 2024, Child A's (1 year old) parent picked her up and received an incident report stating that Child A was having tummy time and bumped her head resulting in a bruise on the head. The report listed the incident as having occurred at 11:00 a.m. but Parent A was not notified until pick-up that afternoon. Compliance Inspector (CI) JD Greer conducted an unannounced investigation and found the allegation to be substantiated based on the following information:
5 CSR 25-500.192(5)(A) which states, "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On September 12, 2024, Compliance Inspector (CI) JD Greer interviewed Managing Member Rachel Kilgore and Caregiver Becca Gooding. Managing Member Rachel stated on September 4, 2024, Child A (7 months old) was being cared for by Caregiver Becca Gooding when Child A hit her head. Rachel was in the room at the time assisting another child. Child A was lying on a floor covering for tummy time when she bumped her head. Rachel did not see Child A bump her head but heard her cry afterwards. Caregiver Becca completed an incident report, but did not notify Parent A (Parent of Child A) of the injury until pick-up later that day. Caregiver Becca stated she was caring for Child A on September 4, 2024. Child A was lying on a small mat and bumped her head on the ground. Child A's head was on the mat. The incident occurred at approximately 11:00 a.m. Parent A picked up at approximately 4:45 p.m.
On October 21, 2024, CI Greer interviewed Parent A (Parent of Child A), who stated on September 4, 2024, Child A hit her head on the ground resulting in a bruise. She was not notified until the end of the day at pick-up.
