On September 19, 2024 the Office of Childhood (OOC) received a report alleging "There are concerns of lack of supervision after a 10-month-old infant fell out of their highchair on September 18, 2024." After conducting an investigation, Compliance Inspector (CI) Caylin Mertz has found this allegation to be substantiated based on the following evidence and rule violation:
5 CSR 25-500.092(1)(C)2 which states "Infants and toddlers. Infants and toddlers who are unable to sit at a table shall have one (1) piece of mealtime feeding equipment for every four (4) infant/toddlers which may include feeding tables, high chairs, infant seats or other safely designed infant seating equipment. Equipment shall be provided which will allow the child to sit comfortably and securely while being fed. Appropriate restraints shall be used."
On September 20, 2024 CI Caylin Mertz conducted an unannounced inspection and walkthrough at the facility. Over the course of this investigation CI Mertz interviewed Director Shelby Sloan, Caregiver Patience Shockley and Parent A (Child A's Mother). Patience indicated at the time of the incident, she was putting Child A in a highchair when another child started choking. She quickly set the tray on Child A's chair and went to respond to the choking child. She did not buckle Child A into the highchair. She must not have gotten the tray on all the way. Shelby indicated she was not present for this incident but did receive an incident report about it. Parent A indicated Patience contacted her and told her that Child A had fallen out of the highchair. Patience told her that Child A was not buckled in the highchair. She has observed children unbuckled in highchairs in the past at the facility.
