Conclusion Summary
On October 16, 2024, the Office of Childhood (OOC) received a report alleging that after on October 9, 2024, reporter's child came home from the facility and had a bite mark across her nose. The daycare did not notify either parent of the incident at the time it happened. Parents have not been notified of things happening before. After conducting an investigation, Compliance Inspector (CI) Curtis West finds this allegation to be substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility: 5 CSR 25-500.192(5)(A). Which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions. On October 16, 2024, CI West conducted an unannounced inspection and interviewed Director Kim Sunarjo, Caregiver Patricia Maya, Caregiver Kristi Smith, and Caregiver Mikayla Collin. Kim stated on October 9, 2024, Mikayla was present for the bite and made the incident report. Mikayla did not advise management, so Parent A (Parent of Child A) was not notified immediately. The incident report was given to the Grandparent A (Grandparent of Child A) at pick up. Patricia stated Mikayla witnessed the bite and wrote the incident report. Mikayla separated the children and cleaned the bite mark on Child A. She thought Mikayla reported the incident to the director. Kristi stated she was not near Child A when the bite occurred. Mikayla witnessed the incident and took care of the incident report. Mikayla stated she is not the main teacher for Child A. She did not witness the bite occur. She heard Child A cry and when she checked on her, she saw the bite mark on her nose. She separated the children and then checked and cleaned the bite area. She wrote the incident report and thought Child A's main teacher would advise the director. On October 16, 2024, CI West interviewed Parent A, who stated she was not notified from the daycare when Child A was bitten on the nose on October 9, 2024. When she picked up Child A from Grandparent A that night, she received an incident report from the daycare stating Child A was bitten on the nose. On October 10, 2024, she called the daycare and spoke with Kim, who stated Child A was bitten on the nose while at recess. Kim stated the incident report was signed by Grandparent A at pick up on October 9, 2024. Kim stated there was a breakdown in communication and apologized for not notifying her immediately. On November 1, 2024, CI West interviewed Grandparent A, who stated when she picked up Child A from daycare, she noticed a nice sized bite mark on her nose that came down to her face. One of the teachers brought over an incident report for her to sign. She took a copy of the incident report to give to Parent A. She has signed incident reports in the past and they would tell her that the parents have been notified. This time they did not state the parents had been notified. The first time Parent A heard about the bite was when she picked Child A up from her.
