On November 12, 2024 the Office of Childhood (OOC) received a report alleging "Child A (2 years old) had bruising on their hands and knuckles and bruising on bite marks on their body." After conducting an investigation, Compliance Inspector (CI) Caylin Mertz has found this allegation to be substantiated based on the following evidence and rule violation:

On November 15, 2024 and December 9, 2024, CI Mertz conducted unannounced inspections and walkthroughs at the facility. Over the course of the investigation, CI Mertz interviewed Assistant Director Shawna Clifford, Caregiver Arianna Bond, Caregiver Nick Ducker and Parent A. Parent A indicated on the date of the incident, she arrived to pick up Child A and noticed multiple bite marks on his arms. She indicated she was told Arianna didn't see it happen and Maggie was not in the classroom. Arianna and Nick both indicated they were sitting in the back of the classroom playing cards with an older child who was upset. They both acknowledge they were not diligent in supervising the remaining children in the classroom. Shawna indicated one incident report was done on the day of the incident when Child A was bit. CI Mertz observed the incident report state Child A was bit by a friend.
CI Caylin Mertz observed video footage dated October 25, 2024. Caregivers Arianna and Nick were observed sitting at a table at the rear of the room playing cards with a child. The remaining children in the class were sitting in chairs in the front of the classroom watching a movie. The room appears to be dark. Child B is observed to bite and hit Child A multiple times. Child A appears to be crying. Child B is observed to hit and bite Child A for nearly 30 minutes. Caregivers Arianna and Nick do not check on the children at all during the clip.
Caregivers failed to provide competent supervision when they allowed Child A to be hit and bitten for nearly 30 minutes by another child.

The facility shall notify all staff members of the violations which were substantiated, and specify the actions to be taken by all staff in order to comply with all licensing violations cited. A copy of the memo, letter, or staff meeting agenda shall be submitted to the Office of Childhood.
