On November 27, 2024, the Office of Childhood (OOC) received an allegation that Cadence Academy Preschool, Beagle had two windows in the gymnasium that were broken and had not been repaired since November 18, 2024. After conducting an investigation, Compliance Inspector (CI) Patrick O'Malley has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.082(2)(A)6., which states: "Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces."
On December 2, 2024, CI O'Malley conducted an unannounced inspection of the facility and observed two broken windows in the gymnasium. Both windows were cracked rather than shattered and neither window was accessible to the children. Designee Benjamin Engle stated that the windows were broken during lawnmowing, when rocks were kicked up by the groundskeeper's lawnmower. Mr. Engle stated that a work order had been submitted for the windows to be replaced.

The facility shall notify all staff members of the violations which were substantiated, and specify the actions to be taken by all staff in order to comply with all violations cited. A copy of the memo, letter, or staff meeting agenda shall be submitted to the Office of Childhood.
