On December 23, 2024, the Office of Childhood (OOC) received a complaint alleging on Friday, December 20, 2024, the preschool children were placed in the Pre-K class while the preschool class was being rearranged. The Pre-K class was not equipped with a diaper changing table, which meant that all preschoolers in diapers/pull ups were not changed and did not have their diapers, wipes, and bedding in that room. Several children went home with soaked clothes. Compliance Inspector (CI) JD Greer conducted an unannounced investigation and found the allegations to be substantiated based on the following information:
5 CSR 25-500.082(4)(A) which states, "A safe diapering table with a waterproof washable surface shall be used for changing diapers. The diapering table shall be located within or adjacent to the group space so the caregiver using the diapering table can maintain supervision of his/her group of children at all times."
On December 31, 2024, CI Greer interviewed Assistant Director John McMillen, Caregiver Cheyenne Mason, and Parent A (Parent of Child A). John stated there is not a diaper changing table in the Pre-K classroom. There are usually no diaper wearing children in the Pre-K classroom. The children were moved into the room while the preschool room was cleaned and re-arranged. Cheyenne stated on December 20, 2024; she was in the Pre-K classroom assisting Caregiver Miranda Terrell. There is not a diaper changing station in the Pre-K classroom. Parent A stated on December 20, 2024; she picked her child up from the Pre-K 1 classroom. There is not a diaper changing table in the classroom.
On December 31, 2024, CI Greer conducted an unannounced walk through of the facility and observed there is not a diaper changing table in the Pre-K classroom.

5 CSR 25-500.182(1)(E)4. which states, "Diapers and wet clothing shall be changed promptly."
On December 31, 2024, Compliance Inspector (CI) JD Greer interviewed Assistant Director John McMillen, Caregiver Cheyenne Masen, Caregiver Julissa Bosley, and Parent A (Parent of Child A). John stated on December 20, 2024, Cheyenne was in the room assisting Caregiver Miranda Terrell. Cheyenne told John that some diaper checks and changes were missed on December 20, 2024. Cheyenne stated on December 20, 2024, she was in the Pre-K classroom assisting Miranda. She does not recall seeing Miranda change any diapers. She did not check and change every child's diaper. Cheyenne arrived in the Pre-K classroom at approximately 12:00 p.m. She left for the day at approximately 6:00 p.m. There were two or three children who were wearing diapers. She didn't think to check or change diapers because they were in the Pre-K classroom and normally those children do not wear diapers. She did not check or change Child A's diaper. Julissa stated on December 20, 2024; she was in the Pre-K classroom helping Miranda put children to sleep. Julissa was covering Cheyenne's lunch break. Julissa did not check or change any diapers during that time. Parent A stated she picked her child up from the Pre-K 1 classroom. She went to the store after work and noticed her child's diaper was the same one from the morning. It was very full and leaking.
