Compliance Inspector (CI) Jessica Graham investigated the allegation that on February 11, 2025, Child A (age 5-months-old), had a bloodied, bruised nose from falling onto a toy at 2:00 PM and the parent was not notified until pick up. This allegation is substantiated based on the following rule violation.
5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On March 3, 2025 director Paige Russom stated that she was told Child A, age 5-months-old, had bumped her nose on a toy causing it to bleed. She was away from the facility during this time but caregiver Anni Johnson called to inform her. Paige will contact parents when accidents happen but forgot to do so this time. None of the caregivers at the center are responsible for contacting parents, only for writing up reports.
On March 3, 2025, CI Graham reviewed the incident report for Child A. It was dated February 11, 2025, with a time of 2:10pm. Anni Johnson was the caregiver that completed the report. The report stated the following:
"Child A was on her belly for tummy time. She had her binky in her mouth when she was playing with a toy and nodded her head down hard hitting the toy with her binky in mouth and hit her mouth and nose. Nose bled and has a mark on the nose. Picked her up and cleaned her up. held ice pack on it. Lots of hugs and loves." The notice to the parents stated it was verbal at pick up time by Anni.
On March 10, 2025, Parent A stated that Parent B picked up Child A around 5:00pm on February 11, 2025, and was told that Child A had bumped her nose on a toy causing it to bleed. Neither Parent A or Parent B were notified of the incident prior to pickup.
