On February 28, 2025, the Office of Childhood (OOC) received a report alleging that at least six times Parent A has picked Child A up and his diaper was very wet. Parent A brings diapers all the time, and they are not going through them like they should if they are changing him often enough. Compliance Inspector (CI) Curtis West conducted an unannounced investigation and found the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.182(1)(D)3. Which states "Caregivers shall be alert to various needs of the child such as thirst, hunger, diaper change, fear of or aggression by other children, and the need for attention."
AND
5 CSR 25-500.182(1)(E)4. Which states "Diapers and wet clothing shall be changed promptly."
On March 6, 2025, CI West interviewed Director Shantelle Tomlin and Caregiver Miah White. Shantelle stated Parent A (Parent of Child A) reached out to her and advised that Child A (2 years old) had not had his diaper changed all day long. She spoke with Miah who stated she did not change Child A. Miah thought that Child A was changed before he transitioned into her classroom. She advised Parent A that the caregivers had dropped the ball that day on changing Child A and it would not happen again. Mia stated that Shantelle spoke with her about Parent A's concern that Child A's diaper was not changed one day a few weeks ago. She did not change Child A's diaper on that day because she thought that Child A's diaper was changed before Child A transitioned into her classroom.
On March 17, CI West received a text message from Shantelle with a screenshot of the app they use for documentation and communication attached. The screenshot showed that on February 14, 2025, Child A went to the bathroom at 12:55 p.m. and then had a nap from 1:00 p.m. to 3:15 p.m. No other bathroom notes were present after 12:55 p.m.
On March 19, 2025, CI West interviewed Caregiver Genisis Lopez, who stated she was the caregiver for Child A in the morning on February 14, 2025. She completed a diaper change on Child A before she left for the day at 1:00 p.m.
