Conclusion Summary
On March 31, 2025, the Office of Childhood (OOC) received a report alleging that Parent A dropped Child A off at 6:30 a.m. and picked him up at 2:21 p.m. Upon arrival, Parent A noticed a bruise under his eye. The teacher, Ms. Hannah, stated they did not know where it came from and assumed that he fell. When you notice a bruise on a child, you are supposed to notify that parent. Parent A didn't get a phone call or a text. Parent A didn't get anything until Parent A picked Child A up at the end of the day. Child A has a black eye, a bump on his head, and a scratch. After conducting the investigation, Compliance Inspector (CI) Curtis West finds the allegation is substantiated. This conclusion is based on the following evidence of a licensing rule violation that occurred at the facility: 5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions." On April 7, 2025, CI West conducted an unannounced inspection and interviewed Director Jami Johnson, Caregiver Hannah Moore, and Caregiver Jennifer McBride. Jami stated that on March 25, 2025, at around 10:00 a.m., she was advised by Hannah that there was a red mark on Child A's face. She instructed Hannah to write the incident report and speak to Child A about how and when he received the red mark. She did not instruct Hannah to notify Parent A. Parent A was not notified immediately after the red mark was noticed. Parent A was not notified about the red mark until pick up at 2:21 p.m. Hannah stated that around 9:45 a.m. on March 25, 2025, she noticed a red mark under Child A's eye. She advised Jennifer and Jami about the red mark shortly after noticing it. Jami advised her to speak with Child A about how and when he received the red mark and then to write an incident report. Jennifer advised her to send a message to Parent A about the red mark. She told Jennifer that she did not feel comfortable sending a message to Parent A. She wrote up the incident report, but she did not notify Parent A about the red mark. Parent A came to pick up Child A at 2:21 p.m. and asked her about the red mark that had turned into a bruise. Jennifer stated that on March 25, 2025, Hannah notified her around 1:00 p.m. that there was a red mark on Child A's face. She did not notify Parent A about the red mark because she was unsure how Child A received the red mark. Parent A was notified about the red mark during pick-up at 2:21 p.m. On April 7, 2025, CI West interviewed Parent A (Parent of Child A), who stated she dropped Child A off at daycare at 6:30 a.m. on March 25, 2025. She picked Child A up from daycare at 2:21 p.m. on March 25, 2025. At pick up, she noticed a bruise under Child A's eye. She asked Hannah if she was supposed to call the parent whenever a bruise like this is noticed. Hannah stated that they were going to contact the parent when they knew how Child A got the bruise. When she dropped Child A off at 6:30 a.m., there was no bruise under his eye. The bruise had to have happened at the daycare. She did not receive any notification of a bruise on Child A from anyone at the daycare.
