On April 09, 2025, the Office of Childhood (OOC) received a report alleging that on April 7, 2025, two junior aides, Sophie Hoffman and Lilly Forrester, age 17 years, were left alone with children and to close the building alone. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations, which occurred at the facility:
5 CSR 25-500.102(1)(C)1. which states "A Junior Aide--" A. "May count in staff/child ratio only if under the direct supervision of an adult caregiver within the same group of children;" B. "Shall not have the direct responsibility for a group of children as the sole caregiver;" D. "Shall not be the sole caregiver on the premises of a child care facility."
On April 10, 2025, CI Eddins interviewed Reporter and Former Director Kelsey Fedor. She stated that Caregiver Ana Sophia Hoffman and Caregiver Lilian Forrester are the only two junior aide caregivers who worked at the facility. On April 7, 2025, Ana Sophia and Lillian were left with the children and had to close the building alone.
On April 10, 2024, CI Eddins conducted an unannounced inspection and interviewed Child B. Child B stated that sometimes he has seen caregivers working alone.
On April 10, 2025, CI Eddins conducted a walkthrough of the facility and observed the facility staff attendance report, which was dated from April 7, 2025, to April 10, 2025. CI Eddins observed that Caregiver Lilian Forrester and Caregiver Ana Sophia Hoffman were in attendance on April 7, 2025, from 2:55 p.m. to around 6:37 p.m. and 6:38 p.m. CI Eddins did not observe Caregiver Cody Brinkman on the attendance report for April 7, 2025. or any other adult staff member in attendance past 3:40 p.m.
On April 15, 2025, CI Eddins interviewed Caregiver Lilian Forrester, who stated that Former Director Kelsey Fedor left her without an adult at the facility on April 7, 2025, or April 8, 2025. She was working with Ana Sophia, who was also 17 years old. Both she and Ana Sophia were left alone with the children for about an hour, from around 4:00 p.m. to 5:30 p.m. They were the only staff members present at the facility during the 4:00 p.m. to 5:30 p.m. time frame.
On April 16, 2025, CI Eddins interviewed Ana Sophia. She stated that on April 7, 2025, at around 4:00 p.m. to 5:00 p.m., Kelsey left herself and Lilian alone with children to close the daycare.
