On June 25, 2025, the Office of Childhood (OOC) received a complaint alleging that Child A (3 years old) was picked up from this facility on June 24, 2025 by Parent A. After Parent A picked up Child A from the facility, she noticed that Child A had a goose egg knot on his forehead. Parent A texted Staff Member Jessica at the daycare who said she had no idea how the incident happened. Parent A was not told about the incident when it happened, and no medical care was given to Child A. Compliance Inspector (CI) Heather Radney conducted an investigation of the allegation and found it to be substantiated based on the following information:
5 CSR 25-500.192(5)(A) - In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions.
On June 27, 2025, Compliance Inspector (CI) Heather Radney conducted interviews with Jessica Curtis, Alauria Wendler, Allison Schram, Child B and Child C. Jessica reported she and Alauria were outside on the playground with their class. They saw Child A crying but didn't see what happened. Child A was standing by the tire swing. There were two kids on the tire swing, so she thought he wanted to be on the swing and that is why he was crying. Alauria reported she and Jessica were on the playground with their class. Child A was crying and she thought the kids were fighting over the swing. Allison reported she got to Child A's classroom during naptime. She saw the bruise on Child A's head when he woke up from nap. It was a quarter-sized goose egg and bruised. It was probably noticeable before nap. Child A told her he hit his head outside earlier. Parent A picked Child A up shortly after he woke up. She got a message from Parent A asking what happened to his head shortly after they left. She asked Jessica and Alauria what had happened to his head. Alauria said he must have hit his head on the swing that morning. She asked them to respond to Parent A's message since they were the ones there when he was injured. Jessica responded to Parent A's message. Parent A should have been notified immediately. They should have noticed the mark on his head earlier.
On July 30, 2025, CI Radney interviewed Parent A. When she picked him up, Child A was awake. She took the picture at the bottom of the stairs and sent the message while she was still there at the daycare. She sent the message to his teachers on the app. Jessica messaged her back and said she thought he might have fallen off the tire swing.
On July 30, 2025, CI Radney received and reviewed a screenshot of the messages between Parent A and the facility. The time stamp reads June 24, 3:51 p.m. There is a photo of Child A's forehead, where a dark blue/purple bruise and scratch is visible above his left eye. Parent A asked, "Do you know how Child A hit his head". Jessica responded "We did not how he hit his head I am so sorry [sad face emoji] He was crying this morning while by the black tire swing. Maybe it hit him but he never came to use and we did not see that bruise. I am so sorry that happened [sad face emoji]".
Based on the information provided, Child A was crying on the playground in the morning. Allison reported the bruise was noticeable following naptime. Parent A was not notified immediately of the injury.
