On June 30, 2025, the Office of Childhood (OOC) received an allegation that: Child A (2-years-old) was left in unchanged soiled diapers (with signs of dried feces) on multiple occasions. Staff Crawford and Ewing were involved in these incidents. After conducting an investigation, Compliance Inspector (CI) Michelle Honea has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.182(1)(D)3: which states, "Caregivers shall be alert to various needs of the child such as thirst, hunger, diaper change, fear of or aggression by other children, and the need for attention."
AND
5 CSR 25-500.182(1)(E)4: which states, "Diapers and wet clothing shall be changed promptly."
On July 1, 2025, Compliance Inspector (CI) Michelle Honea conducted a phone interview with Parent A who stated the following. Child A had dried poop in his diaper when she picked Child A up. This happened on multiple occasions. She has talked to the director about the issues and did not get a proper response. She feels the teacher is not changing Child A properly or before they go home. She is working on potty training Child A and Child A is still going to the facility.
On July 2, 2025, CI Honea conducted an unannounced inspection to the facility and interviewed Director Jessica Quick, Assistant Director Bree Kelley, teachers TeDaria Crawford and Tayja Ewing.
Jessica Quick stated the following. There are no cameras in the classrooms. She has talked to both teachers, TeDaria Crawford and Tayja Ewing, that would deal with Child A at the end of the day. She has concerns of proper supervision with Tayja Ewing. She has spoken to HR about the situation and was informed to document everything with the teacher in question. Both teachers have been written up for letting children go home with dirty diapers. Teachers are supposed to change diapers every two hours and to check before the children go home.
CI Honea interviewed teacher TeDaria Crawford who stated the following. She is a floater and only goes into the room for bathroom breaks. Diaper changes are supposed to go in the app but if the app is not working then it is all documented-on paper. Tayja Ewing is aggressive in manner and does not pay attention to the children in the room.
CI Honea interviewed teacher Tayja Ewing who stated the following. She does not have Child A in the room during the day. At the end of the day the two classrooms are combined around 4:00 p.m. because ratio is low and that's when Child A comes into the room. She does not check the children before they go home because the other teacher will say that Child A was changed before coming into the room. The children that come into the room are not hers and she will not change them.
