On July 8, 2025, the Office of Childhood (OOC) received an allegation that Parent A (parent of Child A) was not notified of an injury to Child A (15-months-old) at time of injury by Elevated Learning Academy LLC staff. After conducting an investigation, Compliance Inspector (CI) Patrick O'Malley has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.192(5)(A), which states the following: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On July 8, 2025, CI O'Malley conducted an unannounced inspection of the facility and conducted interviews with director Labrecia Straub, staff Felicia Garland, and Ke'Turah Wilson.
Staff Ke'Turah Wilson stated that the infant class came in from outside at 10:50 a.m. on Monday, July 7, 2025. Child A's right hand had blood on it. Child A's teacher, Felicia Garland, reported that Child A may have fallen to the ground and popped a preexisting blister. Ke'Turah cleaned and placed a Band-Aid on the wound. Ke'Turah attempted to contact Parent A via phone but did not leave a message. Ke'Turah did not try to call or text Parent A a second time.
Staff Felicia Garland stated that she noticed blood on Child A's right hand as the infant class was coming in from the playground on July 7, 2025. Felicia had noticed a blister on Child As right hand earlier in the day. Felicia thought Child A may have popped the blister by falling to the ground or scraping it on a piece of equipment. Felicia did not observe Child A's injury. Ke'Turah treated the wound and contacted Parent A.
Director Labrecia Straub stated that Parent A was informed of the injury at pick-up, around 2:30 p.m. Parent A did not seem concerned about the injury. Labrecia did not think contacting Parent A was necessary because the injury was old and had just reopened. Labrecia had spoken to Ke'Turah about actually making contact with parents after injuries. The facility did not have outside cameras.
On July 10, 2025, CI O'Malley conducted a phone interview with Parent A who stated that no one from the facility contacted her after Child A's injury. Parent A stated that Child A did not have a blister to his hand on July 7, 2025. Child A was no longer enrolled at the facility.
