On July 8, 2025, the Office of Childhood (OOC) received a report alleging there is a 12x18 inch hole in the wall of Early Preschool 3 that has exposed pipes. The staff keep it hidden by a bookshelf. Compliance Inspector (CI) JD Greer conducted an unannounced investigation and found the allegation to be substantiated based on the following information:
5 CSR 25-500.082(2)(A)6. which states, "Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces."
On July 16, 2025, CI Greer conducted an unannounced walkthrough of the facility and observed in the Early Preschool 3 classroom, an empty bookshelf approximately 40 inches in height in the corner of the classroom. Looking at the top of the bookshelf, CI Greer could see what appeared to be an opening in the wall behind the bookshelf. CI Greer easily moved the lightweight bookshelf and observed a hole in the wall. The hole in the wall was approximately 12 inches by 12 inches and approximately 30 inches from the ground. Inside the wall opening was a black pipe running horizontally.

The facility shall notify all staff members of the violations which were substantiated, and specify the actions to be taken by all staff in order to comply with all violations cited. A copy of the memo, letter, or staff meeting agenda shall be submitted to the Office of Childhood.
