On August 20, 2025, the Office of Childhood (OOC) received a report alleging on August 19, 2025, Child A (2 years old) was picked up from daycare and had an injury. He had a Band-Aid over his ear, but there was no incident report or explanation given when mom asked about it. Compliance Inspector (CI) JD Greer conducted an investigation and found the allegation to be substantiated based on the following information:
5 CSR 25-500.192(5)(A) which states, "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On August 21, 2025, CI Greer interviewed Director Joanna Restivo and Teacher Alma Reyez. Joanna stated she was present on August 19, 2025. Alma noticed a scratch near Child A's ear and near his eye at the end of nap time. Nap time ended at approximately 3:00 pnm. The scratch near Child A's eye was not bleeding, but the scratch on his ear was bleeding. She believes Child A scratched himself during nap time. Joanna cleaned the area and applied a band aid. She should have called Parent A (Parent of Child A), but she didn't remember. Alma stated she is Child A's teacher. At approximately 3:00 pnm on August 19, 2025, she noticed Child A's ear was bleeding. Joanna is notified when a child is injured, and she will reach out to the parents.
On August 21, 2025, CI Greer interviewed Parent A (Parent of Child A), who stated on August 19, 2025, she picked up Child A and Child A had a band aid on his ear and a small mark near his eye. She did not receive a call from the facility about what happened to Child A.
