Conclusion Summary
On September 8, 2025, the Office of Childhood (OOC) received a report alleging that on September 2, Child A's grandparent picked up Child A from school. As Child A's grandparent got to the car, she noticed Child A had a Band-Aid on his arm, and he was saying his arm hurt because his friends pushed him off the slide. At no point did his teacher, Raja, inform Parent A that Child A had hurt himself on the playground. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violation which occurred at the facility: 5 CSR 25-500.192(5)(A) which stated: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions." On September 11, 2025, Compliance Inspector (CI) Cole Eddins completed an unannounced inspection and interviewed Director Sharon Hansen and Assistant Director Valerie Peggie. Sharon stated that she heard about an accident that occurred with Child A from Parent A over the phone on September 8, 2025, at around 10:54 a.m. She observed a small scratch on Child A's elbow, about the size of a dime, on September 8, 2025. She was not sure how Child A received a scratch on the elbow on September 2, 2025, or when it happened. Caregiver Rah'Jai Henderson was Child A's sole caregiver on September 2, 2025. Valerie stated she heard about an accident that occurred with Child A on September 2, 2025, but she was unsure how the accident occurred. CI Eddins interviewed Rah'Jai Henderson, who stated that at around 3:05 p.m., Child A fell off his bike while they were playing outside. He had a small scratch on his elbow. At around 3:15 p.m., Child A's grandmother came to pick him up, and she forgot to tell her about the accident. On September 16, 2025, CI Eddins interviewed Parent A and Parent B. Parent A stated that Parent B picked Child A up from daycare on September 2, 2025, sometime in the afternoon. She heard from Parent B that she observed a band-aid on Child A's right elbow that covered a dime-sized scratch/cut. This was the first she heard about an injury to Child A. She observed a cut that was the size of a dime on his right elbow. Child A told her that his friends pushed him off the slide.
