On September 22, 2025, the Office of Childhood (OOC) received an allegation that: There are weeds growing all around playground-in fence line and in/around play equipment. There is weed barrier/plastic coming up through the gravel. After conducting an investigation, Compliance Inspector (CI) Michelle Honea has found this allegation to be substantiated based on the following evidence:
5 CSR 25-500.082(6)(A)4: which states, "The play area shall be safe for children's activities, well-maintained, free of hazards such as poisonous plants, broken glass, rocks or other debris and shall have good drainage."
AND
5 CSR 25-500.082(6)(A)7: which states, "Areas under and around outdoor equipment shall have continuous maintenance to ensure that the material remains in place and retains its cushioning properties. The resilient material shall be supplemented immediately or replaced as needed."
On September 24, 2025, Compliance Inspector (CI) Michelle Honea conducted an unannounced inspection to the facility and interviewed provider Samantha Frank who stated that she no longer has a maintenance person, and she hasn't had time to get the play yard maintained. She will get the play yard cleaned up.
On September 24, 2025, Compliance Inspector (CI) Michelle Honea observed the outside play yard to have weeds all around the perimeter of the play area that have thorns. CI Honea observed several areas where the black plastic weed barrier was sticking out from under the gravel. CI Honea observed a section of the turf to not be attached to the ground which could cause a tripping hazard.
