On October 3, 2025, The Office of Childhood (OOC) received an allegation of on September 29, 2025, during the 8:00 a.m. hour at JL Mudd elementary, Child A (age 7-years-old), had an incident where a table was knocked over. The table fell on his hand. He went through the entire school day with his left hand swollen. Parents were not notified. Child A went to urgent care and his hand was fractured. On October 7, 2025, Compliance Inspector (CI) Christy Hughes conducted an unannounced inspection of the facility and found the allegations to be substantiated based on the following information.
5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On October 7, 2025, CI Hughes conducted in person interviews with caregivers Chloe Calcaterra and Jakari Hetep, school nurse Sandra Wynn and Child A.
Jakari stated that Child A came to him and told him that the table that he was sitting under against the wall had fallen on him and it hit his hand. He looked at Child A's hand, and he had a small cut, so he placed a band aid on the injury. At 8:45 a.m. he took Child A to the school nurse to get a bigger band aid before Child A went to class. He did not write up an incident report at that time and did not contact Child A's parents. Chloe stated that Jakari was the one who was dealing with Child A.
Sandra stated that Jakari brought Child A in and asked for a bigger band aid. She saw he had a cut on his hand, and she did not check him further. She is not responsible for the children during the Y club. Jakari told her he contacted the parents. Child A was not sent back down to her during the day.
Child A stated that he was sitting under the table by the wall playing cops and robbers. The table fell over hitting his hand and it hurt. He showed Mr Jakari and he put a band aid on it. Mr Jakari took him to the nurse before he went to school and got a bigger bad aid. He did not tell his teacher that it hurt.
On October 7, 2025, CI Hughes conducted an interview via phone with Parent A. Parent A stated that she was never made aware of any injuries to Child A. On September 29, 2025, Child A told her what had happened when she picked him up from school. She took Child A to an urgent care to be evaluated and learned of a fracture in his left hand. The facility gave her an injury report to sign on October 7, 2025, but was dated September 29, 2025.
On October 7, 2025, CI Hughes observed the table that fell on Child A.. CI Hughes also observed the injury report sitting on the table that was dated September 29, 2025. Parent A had signed it.
