On October 15, 2025, the Office of Childhood (OOC) received a report alleging that Child A (21 months old) was picked up from school yesterday with a bite mark on his right arm that appeared to have occurred from another child. He has come home with scratches on his arm, back and face, and bruises on his arms, legs and face. The bruising and scratches occurred within the last week. The daycare did not give a reason for the scratching or bruising and has not followed up with his parents. After conducting the investigation, Compliance Inspector (CI) Curtis West finds the allegation is substantiated. This conclusion is based on the following evidence of a licensing rule violation that occurred at the facility:
5 CSR 25-500.192(5)(A), which states "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On October 21, 2025, CI West conducted an unannounced inspection and interviewed Director Baheja Maaroufi, Caregiver Ammina Ismain, Caregiver Ursela Aliksa, and Caregiver Isabella Herbandson. Baheja stated that she will contact the parents once the incident report is reviewed for accuracy. The parent is not always notified immediately after an accident that results in an injury because all the details around the accident haven't been verified. If the parent is contacted immediately after an injury is observed, they will have questions without answers.
Ammina, Ursela, and Isabella provided consistent information. They stated that complete the incident reports whenever a child is injured. Baheja will contact the parents about the injury. They will speak with the parents at pick up if they have any questions about injuries.
On October 21, 2025, CI West conducted a walkthrough of the facility and observed several incident reports where the parent wasn't notified immediately after Child A was injured.
On November 18, 2025, CI West interviewed Paityn Hughes, who stated that she would always tell parents about injuries a child received during pick up. She would write incident reports and give them to the director for filing. She isn't sure if parents were always notified immediately after a child was injured.
