On October 28, 2025, The Office of Childhood (OOC) received an allegation of on October 27, 2025, Child A (16-months-old), fell of the changing the table while the teacher, Cassidy, had her back turned. On October 28, 2025, Compliance Inspector (CI) Christy Hughes conducted an unannounced inspection of the facility and determined the allegations to be substantiated based on the following information.
5 CSR 25-500.182(1)(A)1. which states: " Caregivers shall not leave any child without competent supervision."
AND
5 CSR 25-500.182(1)(E)3. which states: "The child shall not be left unattended at any time while on the diapering table."
AND
5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions."
On October 28, 2025, CI Hughes conducted in person interviews with director Dawn Branscum and administrative director Marcus Williams and caregiver Brianna Lewis-Scott.
Dawn stated that Marcus brought an incident report to her at 3:45 p.m. on October 27, 2025 about Child A (16-months-old), falling off the changing table due to Cassidy turning her back to the changing table and not having a hand on Child A. She asked Marcus to contact Parent A while she reviewed the video footage. Parent A was contacted at 4:15 p.m. and the incident occurred at 2:09 p.m. She spoke with Cassidy briefly before she left for the day and told her they would speak further in the morning and informed her at that time she would not with children who needed diaper changes until the incident was investigated. She has had concerns that Cassidy seems to get overwhelmed easily so she was moved to a room with smaller ratios and Cassidy has been in that classroom with Child A for about 6 months.
Marcus stated that Cassidy called him at 2:45 p.m. about the incident report. He reviewed the video footage and reminded Cassidy of the proper procedure for changing a child's diaper. He made a call to Parent A at 4:17 p.m. to notify her of the accident and she was taken back and had some suspicions. He assured her that Child A was fine, and that Child A did have a bump on the left side of her head. He has had concerns that Cassidy can be a little "spacey" at times. Cassidy doesn't always have full awareness but never neglectful.
Brianna stated that she heard about Child A falling off the changing table from another employee. She has had concerns in the past with Cassidy walking away from the changing table and she did call Cassidy out on it when it happened.
On October 28, 2025, CI Hughes observed video provided by the facility. The video was approximately 26 seconds in length and does not have audio. CI Hughes observed caregiver Cassidy at the changing table with Child A. She turns to speak with another child not keeping a hand on Child A. Cassie turns back around to do something at the sink still not providing support to Child A on the changing table. Child A starts to sit up grabbing the bar on the table and leans over causing Child A to fall over the side. Child A hit the left side of their head on the floor. Cassie turns around and quickly picks up Child A to comfort them.
On October 30, 2025, CI Hughes contacted the center to try and speak with Cassidy Schwab. Marcus Williams, administrative director, answered the phone and stated that Cassidy Schwab officially resigned from her position as of October 30, 2025.
On October 31, 2025, CI Hughes attempted to contact Parent A at 10:39 a.m. A voicemail was left, and no return call was received.
