On November 13, 2025, the Office of Childhood (OOC) received a report alleging that there were concerns reported for Child A falling out of a daycare chair at the table after an employee picked him up by his body and sat him in the chair and did not get him secure. He then fell out of the chair and sustained a bruise. After conducting the investigation, Compliance Inspector (CI) Cole Eddins finds the allegation is substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:
5 CSR 25-500.092(1)(C)2. which states: "Infants and toddlers. Infants and toddlers who are unable to sit at a table shall have one (1) piece of mealtime feeding equipment for every four (4) infant/toddlers which may include feeding tables, high chairs, infant seats or other safely designed infant seating equipment. Equipment shall be provided which will allow the child to sit comfortably and securely while being fed. Appropriate restraints shall be used."
On November 18, 2025, Compliance Inspector (CI) Cole Eddins completed an unannounced inspection and interviewed Caregiver Shaqwayla Miller, Caregiver Cameryn Batteau, Supervisor Minni Davidson, and Director Evelyn Etienne. Cameryn, Minni, and Shaqwayla stated that although they did not see how Child A got injured, they did observe a cut above Child A left eye on November 7, 2025. Evelyn stated that she heard that Child A hit his head sometime around lunch on November 7, 2025, while in the care of Cameryn and Keyare. She first heard that staff members did not know what had happened to Child A. When she interviewed Keyare about the incident, Keyare originally lied to her and told her that she did not know how Child A got injured. Keyare eventually admitted to being in a hurry and placing Child A down in his seat during lunch. Keyare stated that while she placed Child A down at the table, she did not make sure that Child A was safely and securely fastened. Child A quickly fell forward and hit his head on the table and had a small cut above his left eye.
On November 18, 2025, CI Eddins conducted a walkthrough of the facility and observed Child A's accident report, which stated that Kayare placed Child A in the table too quickly. Child A hit his head on the table. Child A's left side of his face and eye hit the table, which caused his eye to swell with bruising.
On December 5, 2025, CI Eddins interviewed Parent A, who stated that someone from the daycare notified her over the phone about the injury on November 7, 2025, around lunchtime. She did not know the name of the caregiver who picked up Child A and put Child A in the seat quickly for lunch. The unknown caregiver did not make sure Child A was securely strapped into his seat before she left him unattended. Child A fell forward and hit his head on the table and cut his left eye. Child A has a scar on his eye and got a black eye from the injury.
