On November 18, 2025, the Office of Childhood (OOC) received a report alleging that a 3-year-old was left on the playground alone on November 13, 2025. Another staff member had to alert the three staff members that were inside to go get the child. The 3-year-old was outside for 15-20 minutes. After conducting the investigation, Compliance Inspector (CI) Curtis West finds the allegation is substantiated. This conclusion is based on the following evidence of a licensing rule violation that occurred at the facility:
5 CSR 25-500.182(1)(A)1., which states "Caregivers shall not leave any child without competent supervision."
And
5 CSR 25-500.182(1)(A)3., which states "Caregivers shall provide frequent, direct contact so children are not left unobserved on the premises."
And
5 CSR 25-500.082(6)(A)3., which states "Adult supervision shall be provided at all times when children are outside. For children three (3) years of age and above, staff/child ratios may be one and one-half (1 1/2) times the indoor staff/child ratios. The required indoor staff/child ratios shall be maintained on the premises at all times."
On November 21, 2025, Compliance Inspector (CI) Caylin Mertz conducted an unannounced inspection and interviewed Caregiver Riley Stogner, who stated that Child A was left outside for about 15 minutes.
On January 8, 2026, CI West interviewed Director Danae Fiscus, who stated that Caregiver Kaycia Feistel notified her that Child A was left outside alone for about 15 minutes.
On January 12, 2026, CI West interviewed Caregiver Kaycia Feistel, who stated that she was inside for about five minutes when she started to check the younger children's diapers. She realized Child A wasn't with the group. Caregiver Preston Volz went outside to look for Child A and found him unsupervised still playing by himself on the playground.
On January 12, 2026, CI West interviewed Preston, who stated that he was working with Kaycia in the preschool classroom November 13, 2025. Kaycia went to change Child A when she noticed he wasn't with the group. He went back outside to check the playground. He found Child A playing alone unsupervised by the bottom of the slide. Child A was outside alone for about 10-15 minutes.
