On January 7, 2026, the Office of Childhood (OOC) received a report alleging that Child A was enrolled in this daycare without proper medical documentation. After conducting the investigation, Compliance Inspector (CI) Curtis West finds the allegation is substantiated. This conclusion is based on the following evidence of a licensing rule violation that occurred at the facility:
5 CSR 25-500.122(2)(A), which states "The provider, within thirty (30) days following the admission of each infant, toddler or preschool child, shall require a medical examination report signed by a licensed physician or registered nurse who is under the supervision of a licensed physician and completed not more than twelve (12) months prior to admission. The provider may use the department's medical assessment form or the provider may use its own form if it contains all the information on the department's form. The Child Medical Examination Report (Infant/Toddler/Pre-School) form, revised 2021, is incorporated by reference in this rule, as published by the Missouri Department of Elementary and Secondary Education, PO Box 480, Jefferson City, MO 65102-0480 and available by the department at https://dese.mo.gov/childhood/forms. This rule does not incorporate any subsequent amendments or additions."
On January 12, 2026, CI West interviewed Parent A (Parent of Child A), who stated that the daycare doesn't have any of the required medical forms needed for Child A. Child A's pediatrician hasn't completed any medical forms for the daycare.
On January 15, 2026, CI West conducted an unannounced inspection and interviewed Director Vanessa Wills, who stated that Child A has been enrolled since November 11, 2025. She had requested the medical forms be completed from Parent B (Parent of Child A). The medical forms are still not completed.
On January 20, 2026, CI West received an email communication with the medical form for Child A. Medical form was completed and dated January 12, 2026.
