Conclusion Summary
On March 24, 2026, the Office of Childhood (OOC) received an allegation that: On March 23, 2026, Child A (age 18-months-old), was picked up and Child A's diaper was soaked with urine and poop, and Child A had a rash on her bottom. This has happened several times before and was addressed with the director and nothing changed After conducting an investigation, Compliance Inspector (CI) Michelle Honea has found this allegation to be substantiated based on the following evidence: 5 CSR 25-500.182(1)(D)3. which states: "Caregivers shall be alert to various needs of the child such as thirst, hunger, diaper change, fear of or aggression by other children, and the need for attention." AND 5 CSR 25-500.182(1)(E)4. which states: "Diapers and wet clothing shall be changed promptly." On March 31, 2026, Compliance Inspector (CI) Michelle Honea conducted an unannounced inspection to the facility and interviewed Director Kitana McWilliams, teachers Kandace Elsey and Shakara Turner. Kitana stated that diapers are changed 9:00 a.m., 11:00 a.m., after nap time, 2:00 p.m., 4:00 p.m., or as needed. Teachers are supposed to post in the app every diaper change. She has already talked with both teachers and there will be changes made. CI Honea interviewed teacher Kandace Elsey who stated that she was dealing with a couple of other fussing children when Parent A walked into the room. She never checked Child A and did not know she had pooped. She will not let this happen again and will check more often. CI Honea interviewed teacher Shakara Turner who stated that she was not attending to the children. She was pretty sure Child A had been changed before Parent A came in to pick up Child A around 4:20 p.m. She offered to change Child A so Parent A would not have too but Parent A said no. On March 31, 2026, CI Honea observed the online classroom report for Monday, March 23, 2023, that showed the last diaper change for Child A was done at 2:38 p.m. and Parent A picked Child A up at 5:00 p.m. and Child A had pooped. On April 1, 2026, Compliance Inspector (CI) Michelle Honea conducted a phone interview with Parent A who stated that she walked in the room to pick up Child A (around 5:00 p.m.) and saw Child A had poop all over the back of her clothes. She went to the cubby and got a change of clothes for Child A. She checked the app and Child A had last been changed around 2:00 p.m. She talked to the director the next day and was told it would not happen again.
