Conclusion Summary
Compliance Inspector (CI) Jessica Graham investigated the allegation that on Wednesday, April 8, 2026 Parent A picked up her two children, Child A (5 -years- old) and Child B (3- years- old). She was informed Child A had gotten bit. The bite was located on the arm and was already bruising. Parent A stated that no one called her or let her know until she arrived for pick-up. This allegation is substantiated based on the following rule violation. 5 CSR 25-500.192(5)(A) which states: "In case of accident or injury to a child, the provider shall notify the parent(s) immediately. If the child requires emergency medical care, the provider shall follow the parent's(s') written instructions." On April 14, 2026, Parent A said that she found out about Child A being bit when she picked up Child A from the facility. Child A told her that she was bit and told the teacher outside but no one ever called her to let her know. On April 15, 2026, Bekah Roth, director said that Brittany Cooper, caregiver was in charge at the time that Child A was bit. Typically if a child gets hurt they will write a report and the parent will be contacted. Brittany could have gotten busy and forgot to text Parent A about it. Makayla Neal, caregiver said she was not aware of Child A getting bit but knows that any time they see an injury on a child then a report should be written. Makayla stated that the parents get informed at the time of pick up. Jessica McGinnis was outside when Child A was bit. She said that she helped Child A, got her an ice pack and then told Brittany that Child A was bit. She knew that Parent A was told about the bite at pick up but not sure if she was informed before then. Bekah later asked Brittney if she had informed Parent A about Child A's bite mark prior to pick up time and Brittney stated that she did not. On April 15, 2026, CI Graham observed the report of accident, injury and/or emergency medical care dated April 10,. 2026, at 11:30 for Child A, age 5-years-old. The report listed Jessica M as the person responsible for Child A and Brittney as the person in charge. Libby S was listed as observing the incident. The report stated: "While playing on the playground a friend bit Child A on the arm. Ice pack, separated." The report was signed and dated by Bekah Roth, Jessica McGinnis and Parent A. There was no date or time for the method of contact listed for Parent A.
